This is maybe one of the most famous spreads, and to me, one of the most useful to get a general view on a person or a sitaution. Here is my humble interpretation.

Smal Cross: First Card
It shows us the basic situation or problem that caused the reading.
Small Cross: what goes over
It tells us about situations that are present within the first card, both antagonic as well as favorable. It rounds up what
was shown on the previous card.
Past
It shows us facts or people from the past, related to the current situation.
Present
Present sitaution.
Possible Final
It shows us a possible final to the current situation: may either be similar or antagonic to the final resolution, but the
keyword here is POSSIBILITY.
Inmediate Future
A situation that will happen in the near future regarding what was shown on the reading, but that may not last long.
From here we start getting into the querent's psique in a much deeper way:
The Querent
The real stance of the querent regarding the situation that caused the reading.
Environment
What happens to the environment surrounding the querent, whether it's favourable or not.
Hopes and Fears
The querent's secret hopes and fears, that many times have much more to do with the way things evolve than we'd like to admit.
Final Resolution
The way things will evolve, or the direction in which things are going.
